Instructions:

Federal regulations require all Trainees/Interns complete a Final Extension Evaluation at the end of their extension period. The purpose of this evaluation form is to help you reflect upon your program goals at the end of your extension period, as outlined in the Training/Internship Placement Plan you and your host company submitted with your extension application. It is also meant to assist InterExchange in evaluating the efforts of your host company to provide you with bona fide training during your extension. Please complete this evaluation at the end of your extension period and share your comments with your host employer before submitting the evaluation to InterExchange. Thank you.
