This assessment tool is intended to provide the trainee with a final extension evaluation of her/his efforts to meet the goals of the InterExchange International Practical Training Program, as outlined in the Learning Agreement & Training Plan. Please complete this form at the end of the trainee’s extension program.

Please share your comments with the trainee (if possible) before returning this form to InterExchange. It is a U.S. Government requirement that all trainees receive evaluations and feedback from their training supervisor. Thank you.
