Instructions: This assessment tool is intended to provide the trainee with a final evaluation of his or her efforts to meet the goals of the InterExchange International Practical Training Program, as outlined in the Learning Agreement & Training Plan. Please complete this form at the end of the trainee's program and share your comments with the trainee (if possible) before returning this form to InterExchange. It is a U.S. Government requirement that all trainees receive evaluations and feedback from their training supervisor. If you prefer to use your own company's evaluation materials, please send us a copy of the final evaluation form. Thank you.
