Final Evaluation Form - To Be Completed By Trainee
Instructions: The purpose of this
evaluation form is to help you reflect upon your efforts to meet the goals
of the InterExchange International Practical Training Program as outlined
in the Learning Agreement & Training Plan you and your host company designed.
It is also meant to assist InterExchange in evaluating the efforts of your
host company to provide you with a period of bona fide training in your field.
Please complete this form at the end of your program and share your comments
with your host employer before returning this form to InterExchange. It is
a U.S. Government requirement that all trainees complete final evaluations
of their training program. Thank you.
